Advance Directives

Plan Ahead: Document your Personal Health Care Decisions 

Tallahassee Memorial HealthCare is committed to providing world-class care. We want to provide our patients with the treatment they desire. Every patient has the right to determine how vigorously he/she would want to pursue life-sustaining measures when the end of life is near. For this reason, we encourage our patients to discuss these decisions with their families, to designate a person to speak on their behalf, and to put their wishes in writing. When you plan ahead by completing advance directives, you can feel confident knowing that you are in control of your future health care decisions. There are many options. 

Corporate Compliance

In keeping with our TMH I CARE philosophy and particularly in support of our principles of Integrity and Excellence, the Corporate Compliance Office is available to respond to concerns or inquiries colleagues may have regarding fraud, waste, abuse or non-compliance with laws, rules, and regulations. 

Concerns reported to the Compliance Office will be promptly and thoroughly investigated for proper resolution or response. Please contact the Compliance Office at 850-431-2667 with your inquiry or concern. 

Guest Services

It’s important to us that you feel at home during your visit. Our Guest Services representatives are stationed at the main desk in the Dozier Atrium to help you. If you need assistance with directions, notary services, special requests, or information about accommodations, please stop by or call 850-431-1155.

Foreign Language and Sign Language Translators

Our Nursing Operations Office is here to help you with sign language and foreign language translators. Deaf and hard-of-hearing patients and visitors can arrange for sign language interpreters or us a TTY phone. For Non-English-speaking patients and visitors, interpreters are available through the Language Line. For assistance with these services, please call our Nursing Operations Office at 850-431-5304.

Palliative Care

We understand that experiencing a major illness can cause many forms of stress – physical, emotional, social, and spiritual. If you are in need of Palliative Care assistance, our team works with you and your doctor to coordinate a plan of care that focuses on your individual needs. More about Palliative Care

Spiritual Care Services

When you’re in the hospital it’s natural to feel anxious, especially if you’re faced with making difficult decisions. That’s why our chaplains are here and available at any time to help support you and your family, of any religious faith. More about Spiritual Care

Patient Experience

This department provides advocacy and assistance to patients/families and physicians in all care settings and acts as a liaison between patients, physicians, hospital staff and departments to recognize and remove any obstacles to providing high-quality care. Advocates can be contacted Monday through Friday 8 a.m.–5:30 p.m. at 850-431-5488. After hours, contact the Nursing Supervisor by calling 850-431-4983/4984.

Health Information Exchange

What is the Health Information Exchange (HIE)?

A Health Information Exchange (HIE) allows your medical information to be available and viewed electronically by doctors and your medical team members. An HIE is designed to provide quick access to medical records to make treatments more effective and efficient. Your medical information in the HIE will meet HIPAA privacy and security requirements and state laws.

What types of Health Information are shared?

The health information shared in the HIE could include illnesses or injuries you may have had, x-ray reports and lab results, doctor's notes, allergies and medicines you are or have been taking. It is important to note that the information shared on the HIE is not your complete medical record. 

Benefits to you

  • The HIE helps your doctor get more information about you to help with your medical care.
  • The HIE provides quick access to pertinent medical records. Without the HIE, this information would have been obtained by fax or mail.
  • The HIE may limit the need to repeat tests that have already been done.
  • Your medical team will be able to see important information that you may not be able to provide because of confusion, stress or other medical emergencies.
  • Your medical team could see the medicines you are or have been taking and any allergies you may have.

It's your choice

The privacy and security of your information is important to the Tallahassee Memorial Health Information Exchange. Only your medical team members who use the HIE will be able to see your health information with your permission. Your medical information will be available unless you choose not to share your information (opt-out). If you choose not to share your information, you must complete the "Opt-Out" form.
