The Community Health Needs Assessment (CHNA) is a critical tool for identifying the health needs and priorities of our community. By sharing information about the CHNA, you’re helping to ensure that everyone has a voice in shaping the future of our local health services.
Here’s how you can help spread the word:
1. Get Social!
You can easily support the CHNA by sharing content on your social media. Below are suggested materials that you can post across your platforms.

Share this ready-to-go Facebook post and encourage others to participate in the CHNA.
Download and share this post on your Instagram to reach a wide audience!

2. One-on-One: Print and Share Surveys
Help make the CHNA accessible to even more people in your community. Print out CHNA surveys and distribute them to your family, friends and neighbors.
Once the surveys are completed, gather them and contact us at CHNA@TMH.ORG to arrange for a pick-up.
This simple action can make a big difference in ensuring a diverse range of responses!

3. Paint the Town!
Help spread the word by posting flyers in key locations:
Print the attached flyer and place it in your business, organization or around your community.
You can also share the flyer digitally on websites and in electronic newsletters.
