If you are having issues activating your MyChart account, please call our MyChart Help Desk at 850-431-6019.

For the most features, you can download the MyChart app for Android and iOS. You can also access MyChart by logging in above or visiting https://mychart.tmh.org

You can reset your password by clicking on the “Forgot Password?” link above or by calling our MyChart Help Desk at 850-431-6019. 

Once you activate your TMH MyChart account you will be able to link your other accounts by selecting “Link My Accounts.”

Proxy Access

A patient with an active MyChart account can grant proxy access to a caregiver or support person's MyChart account. The patient granting access must follow these steps:

  1. Log in to your MyChart account on a computer at https://mychart.tmh.org or on the mobile app.
  2. Select the Menu icon at the top left corner of your screen.
  3. Under Sharing, select Share My Record.
  4. Select Friends and Family Access, then click on Invite Someone.
  5. Fill out all fields to send someone an invitation to receive proxy access to your account.
  6. Read and agree to the terms and conditions, then check the box and click Send Invites.

The person receiving access will be sent an email invitation. They will need to log in to MyChart and accept the invitation as a proxy.

You can view another individual’s information in MyChart if you are granted proxy access. Tallahassee Memorial HealthCare reviews requests for proxy access carefully before granting proxy access. If granted proxy access to someone's account, you can see their healthcare information, as well as your own, from within the same MyChart account. Adult patients can grant proxy access to others from within their MyChart account. If the patient is unable to grant proxy access from their own MyChart account (i.e. minor child, incapacitated adult, etc.) you can complete a Proxy Consent Form and return it to our of our clinics or Compliance officer to request this service.

Messaging a Provider

In general, you should expect a reply within 2 business days. We do our best to respond in a timely manner. MyChart messaging should never be used for emergencies or urgent needs. 

No, a MyChart message typically does not go directly to your provider. Members of your care team, such as nurses and office staff, will review the message and forward it to your provider if necessary.  You can expect a response if 2 business days. MyChart messaging should never be used for emergencies or urgent needs. Please call your provider’s office if you need to be seen the same day.

Your Health Record

The patient data in MyChart starts on January 1, 2022 and includes your medical chart, visit details, lab results and imaging results. If you need information prior to January 1, 2022, please visit our Medical Records page to learn how to obtain a copy of your full medical record.

You have the right to request that corrections or amendments be made to your medical records if you believe the information is incomplete or incorrect. You may download our Request for Amendment/Correction of Health Information Form and submit it to our Privacy Officer. Visit Medical Records to learn more

Most test results are released to your MyChart account immediately once they are finalized. If you feel that your test results should be available, but you don't see them in MyChart, it is best to call your provider’s office to discuss the test results.

Your provider is able to determine which types of test results are able to be accessed through MyChart. Tests of a very sensitive nature are not released to MyChart.

Yes. Through Share Everywhere you can give one-time access to your health information to any provider with a computer and internet access. Once logged into MyChart, select the Menu. Scroll down to the “Sharing” section and select “Share Everywhere”.

You can also download information about your visits and link your account with other healthcare organizations that have MyChart. To do this, once logged into MyChart, select the Menu. Scroll down to the “Sharing” section and select “Share My Record.”

Both the myTMH patient portal and the Follow My Health patient portal will be discontinued on March 1, 2025. However, your medical record from March 1, 2022 to present day is available in MyChart.
