Physician Referrals

At Tallahassee Memorial HealthCare, we recognize that providers have a choice about the hospital they choose to refer to. We thank you for the confidence you have entrusted in us to care for your patients.

The TMH Patient Transfer Center

Every day and around the clock, the Tallahassee Memorial Hospital Patient Transfer Center works with physicians and other health care professionals to facilitate rapid and safe transfers of patients with all types of illness or injury from healthcare facilities in North Florida, Southeast Alabama and Southwest Georgia. Our one-call advantage can quickly facilitate urgent and same-day admissions to TMH. Our goal is to streamline the referral and acceptance process, making it faster, more efficient and convenient with improved communications.

Transfer Care Services:

With a single toll-free call, you can receive rapid consultation from TMH physicians. With a Transfer Center specialist on the line at all times, coordination of patient transfer and transportation is completed through a streamlined process for all medical services at TMH. This includes inpatient-to-inpatient and emergency-department-to-emergency-department transfers for all adult and pediatric patients.

To Refer or Transfer a Patient:

  • Call 877-864-8762
  • Have your patient's information ready, including name, condition, vital signs, the referring physician's name and the hospital from which the patient is being referred. 
  • You will be kept on the line while the appropriate TMH physician is contacted.
  • The Transfer Center specialist connects the physicians and remains on the line as a "silent partner" until the patient is accepted in transfer. 

Once your patient is accepted to TMH, the Transfer Center will:

  • Coordinate nursing unit and bed arrangements for your patient.
  • Facilitate the transmission of necessary paperwork from the referring hospital to TMH so that only one "report" is required.
