As of March 1, 2025, we have moved to Epic MyChart as the patient portal for Tallahassee Memorial HealthCare and TMH Physician Partners. The patient portal below will be available until April 1, 2025 in a read-only view. It will not contain any information past March 1, 2025. Through the portal you can:
- Access your electronic medical information (for example: results, medications, procedures, diagnoses, consultation notes, discharge summary notes, history and physical exam information, imaging narratives, laboratory report narratives, pathology report narratives, procedure notes, progress notes and other treatment related information from your TMH visit).
- Send medical information to other healthcare providers.
- Connect the myTMH Patient Portal to select health management apps.
