Thoracic Oncology Clinic Referral Instructions for Providers

Who Should Be Referred To The Tallahassee Memorial Thoracic Oncology Clinic?

Appropriate patients for referral into the Tallahassee Memorial Thoracic Oncology Clinic include:

  • Patients undergoing lung cancer screening per Medicare guidelines (listed below) with 4b findings on the Lung-RADS assessment scale.
  • Cancer survivors with suspicious lung or thoracic findings, including patients followed by medical and radiation oncologists, as well as primary care physicians.
  • Symptomatic patients who have suspicious objective and subjective findings on assessment and/or radiologic tests.

Medicare Lung Cancer Screening Criteria

Medicare Part B (Medical Insurance) covers lung cancer screening with Low Dose Computed Tomography (LDCT) once per year if a patient meets all of these conditions:

  • Age 50-77;
  • No signs or symptoms of lung cancer (asymptomatic);
  • A current smoker or has quit smoking within the last 15 years;
  • And a tobacco smoking history of at least 20 “pack years” (an average of one pack – 20 cigarettes – per day for 20+ years).

  • Send an Email

    ThoracicClinic@TMH.ORG with the patient name, DOB and point of contact for referral (name & phone number). Alert patient to referral so they will be prepared when they are called.

  • Submit Referral

    Referral form will be submitted to referring office for completion along with records request.

  • Schedule Appointment

    Thoracic Clinic Navigator will contact patient to schedule appointment and will notify point of contact for confirmation of scheduling.

  • Visit Summary

    After the clinic, referring office will receive summary of visit detailing patient plan of care.
